Koved Care
provides comprehensive Care Management and private-duty care for clients
who are newly diagnosed or in on-going treatment for cancer, are under
Hospice care, or who have chronic pain / symptom management challenges.
As with all of our services, our goal is to help clients find the best
treatment options for them, to optimize their quality of life, and to
minimize hospitalizations.
Our Services
Koved Care provides comprehensive care management and private-duty care
for clients who are newly diagnosed, are in on-going treatment for cancer,
are under Hospice care, or who have chronic pain / symptom management
challenges. As with all of our services, our goal is to help clients find
the best treatment options for them to optimize their quality of life,
and to minimize hospitalizations.
The emphasis with oncology / pain management care management is on advocacy,
with the aim of assisting the client to ask the right questions, find
the right sources for additional opinions, make informed choices with
respect to treatment protocols, and gain access to the particular doctor(s)
with whom they wish to work. Hospice and Palliative Care
Management are available in California and New York, but the Oncology
practice is currently limited to the New York office.
Care Management:
Koved’s care managers provide comprehensive, advocacy-driven oversight
of all aspects of your care. This includes a comprehensive nursing assessment
to determine the best mix of services for you. Specifically, the care
manager will:
- Manage all aspects of care appropriate for the client’s needs,
including safety, mental health, medical and social needs.
- Provide research and logistical support to identify and contact
second opinion resources along with providing available options for
treatment practitioners, protocols, and sites
- Serve as an advocate and coordinator of care with other agencies
or professionals, including doctors, therapists.
- Arrange for, staff and supervise private-duty care.
- Assist when a change in care is required.
Home
Care Services:
Koved Care provides reliable, uncomplicated home care services at competitive
rates. We handle all of the paperwork, pay all related taxes, and pay
for all required insurance coverage.
Our home care services include all customary assistance with “activities
of daily living” (“ADL’s) and are provided by experienced
caregivers, most with CNA or HHA certification. Based on the care plan
that we develop in consultation with you, your family and your doctor,
our care givers can provide assistance with shopping, cooking, eating,
personal hygiene, ambulation and transfers. We can also transport and/or
accompany you for errands, recreational, educational or social activities,
and medical appointments. Finally, we provide medication reminders,
help with range-of-motion exercises, and shift by shift documentation
to your doctor or fiduciary, as requested.
Services are available on a part-time, full-time or live-in basis. For
part-time services we have a 3-hour minimum per visit. For full-time
clients, we will include up to two hours of RN Care Management services
(a $250 value) for assessment and care planning.
How we work with you
We do not charge for introductory consultations, but a Koved Care care
manager is required for all oncology or palliative care cases. Upon
being retained to provide care management services, the care manager
will work with you to develop and initiate an action plan that best
fits your needs. We can tailor any combination of care management and
private duty care coach support that is most helpful for the client.
Services are available on a part-time, full-time, or live-in basis.
The Koved Care Difference
We provide qualified, specialized care management for oncology and palliative
care clients so that we can advocate effectively on your behalf. Typically,
a RN will be the lead care manager in Oncology / Palliative / Hospice
cases.
Koved Care has a multi-disciplinary
care management team that includes both RN's and licensed mental health
professionals.
RN care managers have medical
training and experience that enables them to identify and rapidly seek
treatment for subtle changes in medical status or drug interactions
that could otherwise escalate into crisis situations. By responding
early, we are often able to obtain office-based treatment, avoiding
the disruption and exposure of emergency room visits and hospital stays.
Our licensed mental health professionals provide expertise in identifying
and addressing clients' psycho-social needs, and in assisting them to
access social-service and community resources.
Multiple studies show that clients with professional care managers experience
fewer hospitalizations and emergency room visits.
All of our personal care attendants (care givers) are our employees.
They undergo extensive criminal background, reference and health screening.
They are bonded and insured, and are subject to random drug testing
We believe that it is extremely important to match care givers who fit
the needs, personality and style of living of each individual client.
We avoid changes in staffing and schedules unless requested by the client.
