Our Services

Specialized private-duty services: Koved Care provides comprehensive, specialized private-duty home care / companion services for clients with axis-I psychiatric disorders who require individual support in order to live in non-institutional settings, or who require qualified companions to accompany them on outings. The activities of the care coaches are focused on providing supportive companionship, encouragement and an empathic ear to help clients remember and use the tools they have learned in the course of their treatment. They also provide shift-by-shift documentation, and communicate regularly with the care manager and other members of the care team. They do not provide psycho-therapy or any other licensed services.

Based on direction from the care manager, care coaches can:

  • Assist in researching and arranging recreational, educational or vocational activities as appropriate to the client’s interests and condition.
  • Meet with the client before, during or after their work-day.
  • Accompany the client to professional or social events, and for business or leisure travel.
  • Transport clients to and from day treatment, doctors appointments, etc..
  • Provide all customary assistance with activities of daily living, errands, light housekeeping and meal preparation if required.  Please Note:  in compliance with state regulations, services in New York State and Connecticut exclude hands-on assistance with activities of daily living, ambulation, transfers, etc.
  • Provide medication reminders so that clients take medications as prescribed.

Our Care-Coaches have specific mental health training and experience. Psychiatric care-coaches are typically Psych Tech’s, LVN’s, CNA’s or Aides with several years of acute and/or community-based psychiatric care experience. Some part-time staff may be pursuing graduate Psychology, MFT, LCSW or other counseling-related degrees.

All of our care-coaches are our employees. They undergo extensive criminal background, reference and health screening. They are bonded and insured, and are subject to random drug testing.

We believe that it is extremely important to match care givers who fit the needs, personality and style of living of each individual client. We avoid changes in staffing and schedules unless requested by the client.

Specialized Care Management: Koved Care provides comprehensive care management for clients with axis-I psychiatric disorders.  We have a multi-disciplinary team of care managers, including RN's and licensed mental health professionals.   Our lead psychiatric care manager, Neil Liebert, is a Marriage and Family Therapist with 20 years of acute in-patient and community-based social rehabilitation experience.  The latter best describes the style and purpose of our coaching activities relative to the psychiatric client base.  While Neil functions as lead care manager for most psychiatric clients, he is backed up by our RN care managers.

We include RN's on psych care management teams because they have have medical training and experience that enables them to identify and rapidly seek treatment for subtle changes in medical status or drug interactions.  By responding early, we are often able to obtain office-based treatment, which avoids the disruption and exposure of emergency room visits and re-hospitalization. Numerous studies show that clients with care managers have fewer emergency room visits and hospitalizations.

Specifically, the care management team will:

  • Coordinate with referring parties (Treatment Centers, MD’s, Therapists) to manage all aspects of care appropriate for the client’s needs, including safety, mental health, medical and social needs.
  • Develop and revise care plans based on individual goals, values, personal history, needs, and resources in concert with primary health care providers.  Please note: We refer to licensed agencies for Nursing Assessments in New York State and Connecticut.
  • Serve as an advocate and coordinator of care with other agencies or professionals, including doctors, therapists, etc.
  • Interview, train, and supervise care coaches; arrange and oversee delivery of licensed and skilled services.
  • Prepare records or treatment summaries.
  • Help the client and their family evaluate, and choose among multiple options for care.
  • Assist when a change in care is required.

 

How we work with you.

We do not charge for introductory consultations, but a Koved Care care manager is required for all psychiatric cases. Upon being retained to provide care management services, the care manager will consult with the referring party and the key mental health treatment team members in order to determine the most appropriate and useful way to initiate contact with the client. Following an initial assessment meeting with the client, the care manager will recommend a series of action steps to complete assessment, care planning, housing recommendations (if applicable), and staffing recommendations. Upon acceptance of the care plan, the care manager will assign or recruit staff to the case and Koved Care will initiate those services.

We can tailor any combination of care management and private duty care-coach support that is most helpful for the client. Services are available on a part-time, full-time, or live-in basis.
