What Do Koved’s Care Managers Provide?
Koved’s care managers provide comprehensive, advocacy-driven oversight
of all aspects of care for clients, including comprehensive and continuous
nursing assessments, and thoughtful care planning. In implementing the
care plan, they provide help with changes in living arrangements; crisis
intervention; and arranging and managing private duty services, skilled
services, equipment installation, etc.
Why does Koved Care Have a Multi-Disciplinary
Care Management Team of RN's and Licensed Mental Health Professionals?
RN care managers have medical training and experience that allows them
to identify and rapidly seek treatment for subtle changes in drug interactions
and medical status. By responding early, we are often able to obtain
office-based treatment, avoiding the disruption and exposure of emergency
room visits and re-hospitalizations. Our licensed mental health
professionals provide expertise in identifying and addressing clients'
psycho-social needs, and in assisting them to access social-service
and community resources.
The Koved Care Difference
We provide qualified, experienced care managers who have specialized
training and experience, and proven track records managing cases in
their respective areas of practice. Geriatric clients work with
experienced Geriatric Care Managers, and Psychiatric clients work with
care managers that have extensive psychiatric experience.
Koved’s care managers add value by:
- Performing comprehensive medical and psycho-social assessments to
identify drug interaction issues, additional treatment needs, and
other issues that might escape the attention of the individual specialist
MD’s working with the client. Please Note: In
compliance with state regulations, we refer to licensed agengcies
for medical / nursing assessments in New York State and Connecticut.
- Providing a comprehensive written care plan with concrete recommendations,
assessment results, and an explanation of the rationale for each recommendation.
- Arranging for and overseeing delivery of all services recommended
in the care plan. Examples include:
- Hiring, training and managing staff to provide home care or
other private duty services.
- Arranging for occupational and/or physical therapy assessments
so the care manager can determine the type and amount of rehabilitative
and/or private-duty services that will be most beneficial.
- Arranging for additional skilled and/or ancillary services
to provide rehabilitative support where indicated by the OT, PT
or RN care manager recommendations.
- Researching and arranging social, vocational or educational
programs or activities that will benefit the client.
- Reviewing financial and legal needs and offering referrals
to appropriately specialized service providers.
- Recommending, researching, and facilitating placement in a different
living situation (e.g., Board and Care, Assisted Living, etc.), if
such an arrangement is preferable to remaining in the home.
- Facilitating and coordinating communication among the client’s
entire treatment team, and between the client and the entire treatment
team.
- Providing (or arranging for) on-going continuous assessment to identify
subtle changes in condition, and facilitating rapid access to (non-emergency
room) treatment so small problems don’t escalate to crises.
- Providing crisis intervention and management, should such an event
occur.
- Providing support to long-distance care givers.
